The Chronic Care Registered Nurse (RN) will assess patients based on the patient's own evaluation and responses to questions as well as follow up questions to triage and make decisions best for the patient.
This role will work with care coordinators and providers to deliver services to patients as part of our virtual care team.
Additionally, the RN may refer the patient to a provider if needed, schedule an appointment, or recommend another treatment plan.
Roles/Responsibilities Direct Patient Care Assesses patient to determine their understanding of their chronic condition and identify potential care and/or provider gaps Provides counseling and education regarding diabetes management, hypertension management, and smoking cessation in coordination with providers during enhanced medical visits Accesses patient records and searches a database of possible conditions and treatments, and documents telephone encounters, education, and outcomes Assesses and triages immediate health concerns Clinical Administration Provides clear patient information and education for handling their situation Evaluates patients over the phone to determine between emergency care or regular doctor visit is needed Provides ongoing training and support to care coordinators and others to assure effective and appropriate patient services through the VCT Patient Education Serves as a role model and clinical resource across the continuum of care to influence organizational standards related to patient safety and care delivery Acts as a resource for patients by providing education and guidance to support positive health outcomes, including lab tests, results, medication functions Initiates patient education plans according to individualized needs, including patient and family instructions Requirements Education/
Experience:
High school diploma or GED required; bachelor's in nursing preferred.
Valid CA RN license required.
Valid BLS certification required.
Three years of RN experience providing nursing assessment, triage experience required; telehealth triage experience preferred.
Skills:
Bilingual (English/Spanish) preferred.
Reliable transportation for traveling to multiple NHC sites.
Excellent verbal and written communication skills, including superior composition, typing and proofreading skills.
Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.
Ability to successfully manage multiple tasks simultaneously.
Excellent planning and organizational ability.
Ability to work as part of a team as well as independently.
Ability to work with highly confidential information in a professional and ethical manner.
Neighborhood Healthcare requires employees to be fully vaccinated (including a booster dose) against COVID-19 and demonstrate proof of vaccination/booster upon hire or receive approval for an exemption based on a medical reason, disability or religious belief.
Proof of booster dose will be contingent upon booster eligibility.
Pay range:
$48.
00 - $57.
00 per hour, depending on experience.
This role is eligible for a $2,500 signing bonus.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
