Primary Care Physician - San Diego

Vivo HealthStaff is recruiting for a Primary Care Physician in North County, San Diego.
The Primary Care Physician is responsible for providing primary care and acute illness evaluation and treatment to patients.
They will perform comprehensive assessments on patients, collaborate with the IDT to develop care plans, and actively participate in coordinating 24-hour care delivery.
The physician will work collaboratively with other healthcare professionals to ensure effective care coordination.
They will also oversee the use of medical specialists and inpatient care for patients.
The physician will supervise and provide support to Nurse Practitioners and Physician Assistants.
Essential FunctionsoProvide primary care and acute illness evaluation and treatment for patients.
oPerform comprehensive assessments on patients and collaborate with the IDT to develop individualized care plans.
oCollaborate with Community-Based Physicians and the IDT to ensure effective care coordination.
oPlan and coordinate discharges for acute, short-term, and long-term placements in conjunction with the IDT.
oSupervise and support Nurse Practitioners and Physician Assistants.
oOversee the use of medical specialists and inpatient care for patients.
oParticipate in rotating night and weekend call schedules.
oSupervise all care of patients in the nursing home, providing regular visits as necessary.
oCollaborate with the Medical Director and clinical staff to review outcomes, utilization data, and make policy and procedural improvements.
oProtect privacy and maintain confidentiality of all company procedures and information about employees, patients, and families.
oFollow organizational policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
oMaintain professional licensure and certifications within the scope of practice.
Education and CertificationoSuccessful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
oMD or DO degree.
oBoard Certification in Internal Medicine or Family Practice.
oAdditional certifications in Geriatrics, Hospice and Palliative Care, Addiction Medicine, or Pain are desirable.
oActive California physician license without restrictions.
oCurrent Basic Life Support (BLS) training.
oState CDS Certification and DEA Registration.
oCurrent driver's license and proof of auto insurance.
Skills & ExperienceoCurrent knowledge of chronic care/geriatric issues and best practices.
oCurrent knowledge of behavioral health and substance use care.
oAbility to work well in a managed care environment with an interdisciplinary team (IDT).
oAbility to build strong working relationships with other providers and the care team.
oCreative and enthusiastic commitment to improving experience, health, and value for patients.
oAbility to process information accurately and timely.
oExcellent clinical, organizational, and communication skills in settings with frail, elderly patients, their families, and the IDT.
oWorking knowledge of relevant regulations preferred.
oBilingual Spanish-English preferred.

The check will bounce and the victim is left responsible.
