Non-exempt General Description:
Under the direction of the Program Coordinator, responsible for teaching adults with developmental disabilities primarily in the areas of community access, vocational skills and mobility.
Responsible for goal development, goal implementation and program documentation.
SUPERVISOR:
ADC or HAB Program Coordinator Supervises:
N/A Essential duties:
Provide daily training and supervision of consumers involved in Center-based and community- based training.
Assist consumers in the development and implementation of individualized goals.
Track consumer progress, document results and assist with assessment of skills.
Assist with consumer's personal care and feeding.
Maintain ongoing communication with Program Coordinator regarding any consumers needs and conflicts that may arise.
Supervise the work/training activities, performance, production and behaviors of consumers.
(HAB) Supervise the progress on goals and behaviors of consumers.
(ADC) Maintain accurate daily records related to consumer wages (tally sheets and time cards) and annual goals.
Assist in coordinating the operations of the production department, ensuring production quality and deadlines are met as needed.
(HAB) Ensure all jobs and corresponding paperwork are completed according to standards and within deadlines.
Ability to lift and transfer a consumer and/or materials from 50 - 100 lbs.
, pushing and/or pulling up to 250 lbs.
SPECIFIC DUTIES AND
Responsibilities:
Assist in maintaining a unified and cohesive program by carrying out the following duties:
Under the direction of the Coordinator, conduct inventory of contract materials and help with the distribution to work stations.
(HAB) Process and organize bulk mail contracts according to postal regulations as assigned (zip code sorting, bundling, sacking, tagging and paperwork.
(HAB) Provide training and ensure safety to consumers in the community and at the Center.
Training to focus on community access skills, recreation and leisure skills, safety skills and mobility skills.
Provide training to and ensure the safety of our consumers at the Center.
Assist with classes and work one on one with our consumers.
Perform and assist with consumer assessments, both in the community and at the Center.
Assist with the development of consumer goals and monitor goal plan implementation and progress.
Adjust plans or trainings as appropriate to ensure consumers success.
Participate in staff meetings, consumer meetings, in services and training as needed.
Responsible for the following program documentation:
Write out goal plans and document progress.
Document consumer assessment outcomes.
Summarize general consumer performance and attitudes in monthly chart ID notes.
Ensure time cards and tally sheets are accurately completed on a daily basis.
(HAB) Write incident and seizure reports as necessary.
Assist in other aspects of the program:
Assist with toileting and feeding at the Center and in the community.
Perform building duties as assigned, including bus duty and classroom clean up.

Qualifications:
Education and
Experience:
High School Diploma or equivalent.
Possess an aptitude for basic math and determining percentages.
Six (6) months of experience working with individuals with a wide rage of developmental disabilities.
FA/CPR certified or the ability to be FA/CPR certified.
Knowledge of positive behavioral change principles.
Communication Skills:
Ability to communicate effectively and respectfully with co-workers and consumers.
Ability to accurately document consumer information, (e.
g.
assessments, goals, etc.
,) within an established format.
Ability to direct consumers and staff.
Ability to make a positive presentation to consumers, their families, care providers, employers, and the general public.
Reasoning & Mental Ability:
Ability to define problems, collect data, establish facts, and draw conclusions.
Ability to respond respectfully to interpersonal conflicts and problems.
Ability to create and implement new training methodology.
Ability to resolve problems, recall details and be able to concentrate.

Physical Ability & Stress Level:
Ability to lift and transfer consumers or materials from 75-100 lbs.

Work Environment:
Center-based workshop and various community-based environments.
High noise and activity level.
Must have access to reliable transportation to attend meetings, conferences and transport consumers.
Possess a valid drivers license, proof of auto liability insurance, and be able to use the Center van to transport consumers when necessary.
Exposure to blood born pathogens could occur during restrooming, feeding, or when administering first aid to consumers.
Hepatitis B vaccine is offered to all employees at no cost to employee.
TB Test, health screening and a LiveScan background clearance are required for employment with United Cerebral Palsy Association of SD County.
